Working in partnership with the families Primary Care Team
We offer support to children with life-limiting conditions from birth to their 18th birthday within LauraLynn and the child’s family home. At LauraLynn Children’s Hospice we have an experienced Consultant Paediatrician, registered nursing care 24/7, a GP on call 24/7 service and a range of clinical therapies. Our LauraLynn@HOME care programme delivers hospice care within the family home and is delivered by a team of experienced children’s nurses and healthcare assistants.
Our Palliative Care
is planned and delivered through the collaborative efforts of a multidisciplinary team that includes the child, family, care team, external partners and service providers. Once a child has met the admissions criteria, we will develop an individualised Care Plan following admission and a Key Worker will be appointed to the child and family. All our care and support is free to families. Care is delivered by LauraLynn House in a ‘Home away from Home’ environment and by LauraLynn@HOME in the child’s family home.
Our Residential Care
is provided to children/young people who have been with us for a considerable time and we remain committed to the provision of care to them for as long as is required. In line with national policy, we no longer plan any further admissions to this service.
